Statement of Faith/Standard of Conduct

All students need to read through the Statement of Faith (what we believe) and our Standard of Conduct (what we ask students to submit to while a student) and send it in with a signature signifying willingness to abide by both as a student.

Testimony Worksheet

All students are expected to write their testimony. Please follow the instructions carefully and complete all required sections before you send this form in.

Pastor Reference

Please give this form to your pastor, youth pastor, or other church leader that knows you the best. Please note that references cannot be completed by family members.

Non-Pastor Reference

Please give this form to an adult that knows you well and has observed you in a work or educational setting. Please note that references cannot be completed by family members.

Transcript Request Form

Students who attended public or private school should use this form to request transcripts be sent to the Bible Institute for review. All students need to have a second transcript indicating high school graduation sent before they will be able to enroll as a student.

Home School Verification Form

Students who completed a home education program need to submit this verification form along with their final transcript in order for it to be considered official. Initial transcripts do not need to have a verification form, only final transcripts.

For home school families who have not assembled a high school transcript before, this sample form offers a simple format to help in the process.

Health Form

All students are required to submit a health form in order to attend school. Please follow the instructions carefully and submit as soon as possible. All vaccinations need to be on official documentation or have a doctor signature to be considered official. Please attach copies of any insurance cards as well.

Sports Physical

Only students intending to participate in our organized sports ministry teams need to submit this physical form in addition to their standard health form.
